Prerequisites for Requesting a Program Change
Provider Evaluation Required: All program or medication changes must be approved by a licensed provider.
Paused Subscription: When you submit a request, your current subscription will be paused until your provider visit is completed.
Authorization: You must confirm and authorize billing adjustments and acknowledge that pending refill orders cannot be canceled or modified once processing has begun.
Steps to Request a Program or Medication Change
1. Access the Program Change Form
Log in to your Shed Member Portal: portal.tryshed.com
Go to My Treatments → Manage My Subscription → Request a Medication Change
2. Complete the Request Form
You’ll be asked to provide:
Desired medication and dosage
Current weight
Reason for the requested change
Before submitting, you must confirm:
“By submitting this form, I confirm and authorize a charge to my current payment method. My existing prescription will be paused for 21 days, allowing adequate time for my change request to be processed.”
“I understand that if a refill is already in progress, this change will apply to my next order. Shed is unable to cancel or modify orders that are currently processing or preparing for shipment.”
3. Provider Appointment
After submitting your request:
Schedule a virtual provider appointment through your Shed portal.
Your provider will review your request, health details, and determine if the change is appropriate.
4. Provider Review and Decision
Your provider may issue one of the following outcomes:
Pending Approval (awaiting provider review)
Approved for requested medication
Denied – Approved for alternate medication
Denied – Stay on existing medication
Denied – Stop current medication
Once a decision is made, you will be notified in your portal, and your subscription will be updated accordingly.
